The compromised data reportedly includes dates of birth, city, state, gender, ZIP codes, addresses, language, first and last names, member IDs, phone numbers, customer names, email addresses, shipment details, transaction IDs, doctor names, application names, integration point information, organization email addresses, and other patient-related records.
